access to my home. By completing and submitting this 'Reschedule Warranty
Request' form, I understand that the JW Customer Relations Team will close any
and all open service requests on my home. I understand that a John Wieland
Representative will contact me on the date I request to set an appointment for
warranty service. Warranty work will not be performed on that date.
